It is mostly random as the umbilical cord is clamped well away from the navel and does not actually affect the shape of the belly button. The shape and size of the belly button depend entirely on the way a child’s tummy heals after the cord falls off. The belly button takes shape on its own. It is however determined by how the umbilical cord was attached during development inside the womb and not where it was cut.
